COPYRIGHT:
All teaching materials which make up the Basic
Floral Design Distance Learning Program, including
Lesson Plans, Instruction Design, Curriculum, Study Guides, Training DVDs, Floral Design
Training
Techniques, Floral Designs / Arrangements, Quizzes / Exams, Instruction Methods and
Techniques
are the intellectual property of Floral Design Institute, Inc., and are © 1988 - 2008,
Floral Design
Institute, Inc. ALL RIGHTS RESERVED
Permission is granted to an exclusive single user for
the use of these materials for the sole purpose
of completing the Basic Floral Design Distance Learning Program. Any other use,
including, but not
limited to; reproduction, distribution, republication, and classroom training is strictly
prohibited without
prior written permission of Floral Design Institute, Inc.
GENERAL RELEASE:
I, the undersigned, hereby voluntarily and knowingly release from liability and agree to
hold harmless
Floral Design Institute, Inc. and it’s owners, their agents, employees,
representatives, successors and
assigns, from and against any and all claims and liabilities of any type whatsoever,
including attorney
fees, which in any way arise out of, or are related to, either my participation in any
Floral Design
Institute training or my use of the Institute’s tools, equipment, flowers, foliages
and supplies.
I fully understand this to be a voluntary exercise, and I
understand that it poses inherent risks to my
health and safety. Having been so apprised, I elect to and do assume all risk for claims
heretofore of
hereafter arising, known and unknown, arising from the subject of this release and I
knowingly and
voluntarily expressly release the above stated parties liability for claims arising out of
such a matter.
This release shall be binding upon my heirs, successors, personal representatives and
assigns.
I have read all four pages of this enrollment form
including; Course Information, Payment Information
Refund Schedule, School Policies, Copyright, and General Release. I hereby agree
without exception
to all terms, requirements and restrictions included within this enrollment.
